Business/Corporation Certificate of Insurance

A Certificate of Insurance(COI) is needed for the Forest Preserve and most Chicago Park District equipment rentals. If you are having your event at a location and they require proof of insurance, the COI is what will be needed. Please write into the customer notes at checkout, the verbiage that should go on the certificate (that information will be provided by the Forest Preserve, Park, or personnel of the location in which proof of insurance is needed). The COI is NON-REFUNDABLE. 

*The turnaround time for a COI is 48-72 hours (2-3 business days). In the event you need a COI within 24 hours, please give us a call to see if we can accommodate your order before check-out. The COI will be delivered to the email address that we have on file.

Please ensure the person booking the order is the person on the permit.

THE COI ANDTHE PERMIT MUST BE IN THE SAME NAME.
